Job Summary:
The Community Health Worker (CHW) will further the Seager Memorial Clinic’s mission of
providing free, easy-access, non-judgmental, charitable medical, dental, vision and mental
health care to the underserved, by supporting clinic operations and connecting patients with community resources. The CHW builds trust with underserved communities and seeks to understand and minimize barriers to accessing needed services.

Primary Responsibilities:
Clinic Operations:

  • Assist patients in scheduling appointments for diagnostic testing and specialty care, including required documentation to qualify for diagnostic services.
  • Follow and manage external medical referrals and obtain records & results.
  • Greet patients and set clear expectations.
  • Connect patients with local providers of community resources and services.
  • Connect insured patients needing vision or dental care to find a provider that accepts their insurance.
  • File all patient results and keep their charts up to date with new information.
  • Keep volunteer providers apprised of patients needing extra care.
  • Oversee special projects as identified by the Executive Director and/or Clinic Manager.

Secondary Responsibilities:
Community Outreach:

  • Table at the Reentry Fair on Tuesdays from 12:45-2 PM.
  • Represent the Seager Memorial Clinic at monthly Weber County Community Reentry Coalition meetings.
  • Represent the Seager Memorial Clinic at bi-monthly Ogden CAN meetings.
  • Promote Seager Clinic at special events and tabling events in the community as
    well as on social media platforms.

Required Education:

  • High school diploma (required)

Skills & Experience:

  • Bilingual Spanish (strongly preferred)
  • Ability to travel throughout the organization’s service area (required)
  • Strong communication skills (required)
  • Strong organizational skills
  • Ability to identify problems and find solutions
  • Experience working with diverse communities
  • Experience in community outreach and education
  • Experience with Google Workspace, Zoom, and social media platforms

Compensation:

  • $23/hour, 30 hours per week ($35,880/year), plus up to another $5,000/year in bonuses
    based on accomplishment of specific defined goals.
  • Hours are fixed and in person on Mondays, Wednesdays, and Fridays from 2-6 PM and
    Tuesday from 12:45-2 PM, closed holidays. The remaining hours are flexible and can be
    done off-site.
  • Personal laptop will be provided.
  • Employees are eligible to receive Paid Time Off (PTO) to cover required work days in the
    clinic (3 days per week). PTO may be used for vacation, personal time, illness, or any
    other approved time away from work.

Accrual Schedule

  • 0-12 Months of Employment: Employees receive 6 clinic days (equivalent to 2 weeks) of PTO
  • +1 Year of Employment: PTO will increase to 9 clinic days (equivalent to 3 weeks) per year
  • Use: PTO must be used to cover scheduled time off for vacation, illness, or other personal reasons. Employees are encouraged to schedule PTO in advance when
    possible, in accordance with clinic scheduling needs.
