The Licensed Practical Nurse (LPN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of patients. Qualifications:

• Candidates must be licensed and registered in the state of Utah.
• Basic Life Support Certification (BLS) for healthcare providers.
